Pacific Care in Hoquiam, WA, is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. As part of this amazing team of clinicians, you will have opportunities for training and continuing education in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehabilitation, and respiratory/COVID recovery programs. Duties: 
  • Effectively screens/evaluates patients with communication, cognitive or swallowing disorders and develops appropriate plan of care following all regulatory and clinical practice standards.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with communication, cognitive and/or swallowing disorders per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the speech disorders.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. 
Qualifications: 
Licensed Speech-Language Pathologist
This position is open to SLPs at all experience levels. New Grads Welcome!

Duties:

  • Utilizes standardized assessments and evidence-based practice to support clinical interventions.
  • Provides rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
  • Administers various procedures as part of the rehabilitation plan including but not limited to:  ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; etc.
  • Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
  • Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.
  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.
  • Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.
Qualifications:
  • Licensed Occupational Therapist
  • This position is open to OTs at all experience levels.
Pay Rate: $40-$55/HR

  • Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.
  • Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
  • Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.
Qualifications: 
Licensed Physical Therapist Assistant 
Prior experience in a skilled nursing setting is preferred but new grads welcome!
 
Pay Rate: $30-$40/HR.

Qualifications:
  • Must be a graduate of an accredited Occupational Therapy Assistant program 
  • Must hold current and active State license/registration where applicable
